Waiver Statement:

I agree and acknowledge that I am undertaking participation in the MOSHAPE Convention events and activities by my own free and intentional act, and I am fully aware that possible physical injury might occur to me as a result of my participation in these events. I give this acknowledgement freely and knowingly and that I am, as a result, able to participate in the convention events and I hereby assume responsibility for my well-being. I also agree not to allow any other individual to participate in my place. MOSHAPE plans to take photographs and video at the conference and reproduce them in educational news or promotional material for print, electronic or other media, including the website. I grant MOSHAPE the right to use my image, photography and biography for such purposes. All postings become the property of MOSHAPE. Postings may be displayed, distributed, or used by MOSHAPE for any purpose.

IMPORTANT NOTE: Cancellations must be submitted in writing to moshape1925@gmail.com and received prior to November 1, 2026 and will be assessed a $30 cancellation fee. No refunds after November 1, 2026.

I also understand that only registered individuals are allowed to participate in the convention sessions. Due to insurance reasons, minors are not allowed to actively participate in sessions.
