I agree and acknowledge that I am undertaking participation in the MOSHAPE Convention events and activities by my own free and intentional act, and I am fully aware that possible physical injury might occur to me as a result of my participation in these events. I give this acknowledgement freely and knowingly and that I am, as a result, able to participate in the convention events and I hereby assume responsibility for my well-being. I also agree not to allow any other individual to participate in my place. MOSHAPE plans to take photographs and video at the conference and reproduce them in educational news or promotional material for print, electronic or other media, including the website. I grant MOSHAPE the right to use my image, photography and biography for such purposes. All postings become the property of MOSHAPE. Postings may be displayed, distributed, or used by MOSHAPE for any purpose. Cancellations must be submitted in writing to mdriemeyer@lindberghschools.ws and received (7 days) before the convention. Convention registration cancellations received on or before October 15, will be assessed a $10 processing fee; between October 16 and October 31 a $20 processing fee will be assessed; No refunds given after November 1, 2025. If you are paying by Purchase Order you will be billed for the above cancellation fee.
